About This Game

The idle game that takes medicine seriously.

You start with one exam room and a patient on the table. Eight hospitals later, your medical empire spans the country and runs itself.

Diagnose patients across actual medical scenarios. Hire specialists. Unlock departments. Fund research. Respond to crises that hit a working hospital: VIP arrivals at 2 AM, equipment failures, regulatory audits, ransomware on the EMR. Your team treats patients between your sessions. Offline progress accrues for up to 24 hours.

The win condition: a fully autonomous medical empire across eight hospitals in eight cities, from Boston to Los Angeles.

Eight departments, each one runs itself

Unlock Outpatient Clinic, Emergency Department, Operating Room, Intensive Care Unit, Specialty Center, Pediatrics Ward, Psychiatric Unit, and Rehabilitation Center. Every department has its own caseload, passive income rate, and staff that keep working between sessions.

A research tree with consequential choices

31 research nodes across seven branches. Specialize in diagnostics, workflow automation, complication reduction, or revenue. 47 upgrades across four tiers reshape the economy at each stage of growth.

Eight hospital identities

At each empire expansion, choose the kind of hospital you want to run. Level I Trauma Center. Academic Research Hospital. Children's Hospital. Integrated Mental Health Center. Each one changes which cases dominate, which bonuses compound, and how the run plays.

A team of named physicians

Hire 15 staff archetypes, from medical assistants up to a Chief of Medicine. Earn 17 named Consultant physicians whose bonuses carry across every empire expansion. Dr. Sarah Chen. Dr. James Okafor. Dr. Priya Patel. Once they join, they stay.

Crises with consequences

30 event types interrupt the routine. Pharma reps in the cafeteria. Joint Commission audits. Press conferences after a sentinel event. Each event is a paused decision with branching outcomes. Reputation tracks how the community sees your hospital, and shapes which cases walk through the door.
