At each empire expansion, choose the kind of hospital you want to run. Level I Trauma Center. Academic Research Hospital. Children's Hospital. Integrated Mental Health Center. Each one changes which cases dominate, which bonuses compound, and how the run plays.

A team of named physicians

Hire 15 staff archetypes, from medical assistants up to a Chief of Medicine. Earn 17 named Consultant physicians whose bonuses carry across every empire expansion. Dr. Sarah Chen. Dr. James Okafor. Dr. Priya Patel. Once they join, they stay.

Crises with consequences

30 event types interrupt the routine. Pharma reps in the cafeteria. Joint Commission audits. Press conferences after a sentinel event. Each event is a paused decision with branching outcomes. Reputation tracks how the community sees your hospital, and shapes which cases walk through the door.
