Appointed by the royal court to a remote village, you begin your journey as an unknown doctor. Build your reputation from a suspected quack to a reputable Plague Doctor, all while witnessing the spread of the plague day by day.

Step outside the safety of the village walls into an open world filled with natural resources. Travel to lush fields, navigate deep caves, and traverse dense forests to locate rare medicinal herbs, mushrooms, and minerals.

Identify and refine your gathered ingredients. Experiment with mixtures to discover potent medicinal properties, creating treatments that range from simple potions and pills to complex ointments and experimental injectable serums.

Every patient tells a story. Listen to their plight, examine their bodies for symptoms, and establish a diagnosis. Your treatments require steady hands, ranging from basic remedies to invasive surgeries and medieval practices.

Will you uphold the Hippocratic Oath, or decide who gets to live and who doesn’t? Choose carefully. The world responds dynamically to your success and failure. With every untreated patient and every death, the environment decays: homes fall into disrepair, fires break out, the weather turns turbulent, and nature itself withers.

Immerse yourself in a village where every NPC has a unique life, schedule, and behavior. Get to know them as you treat them, or watch them succumb to sickness, or perhaps, to your own malpractice.

Whether you bear the burden of the plague alone or share the task in 4-player co-op, the work never ends. However, when the pressure mounts, you can always find a moment of peace in the tavern, play the lute, or throw some dice.
