“I am Leandro, a indie solo developer from Argentina Buenos Aires who started creating games and released our first County Hospital 6 years ago. What began as a small personal project quickly evolved. We wanted to push the limits of Unreal Engine, and we are proud to have been among the first developers to showcase its photorealistic capabilities and multiplayer


We chose Early Access for several reasons:

“We anticipate that County Hospital Cautery will remain in Early Access for several years. The duration depends on community feedback and the improvements we plan to introduce. Our goal is to let the game grow step by step, with regular updates during Early Access and even after the official release. The final version will be significantly more complete, with many additional features, content expansions, and refinements that will enhance the overall experience.”

How is the full version planned to differ from the Early Access version?

“County Hospital Cautery is already in an advanced state, with high-end graphics and solid, intuitive gameplay. However, we still have a lot of work ahead of us to bring the game to its full potential.
Our vision is for the final version to be richer, more polished, and more complete.”

“The current Early Access version of County hospital Cautery is already well advanced. The core experience is fully playable, stable, and provides a strong foundation that will continue to grow over time.

Gameplay: An immersive, realistic (but not a simulation), and intense FPS focused on teamplay and tactical decision-making.
Graphics: Photorealistic visuals powered by Unreal Engine 5 and Megascans, using next-generation rendering technologies.
Game Modes: One game mode, 4 players working together on a coop missions and then its a 4vs1 deathmatch providing an original new concept of game

Weapons: Original mechanic using flamethrower kicks and grenades.
Maps: Multiple environments such as abandoned labs, kitchen, bathroom, and more.”

COUNTY HOSPITAL: CAUTERY is a 4-player cooperative sci-fi horror experiment with a twist and flamethrowers. Complete a multi-stage mission to eradicate a biological outbreak, but beware: success triggers a hidden protocol, turning one of you into an specimen that hunts the rest.

▶ SINGLEPLAYER  or **STRATEGIC 4-PLAYER CO-OP**

Inmerse in a solo campaing mode or work as a team using flamethrowers to complete 8 interconnected objectives: photograph organisms, restore power, retrieve key items, activate systems, and cleanse the facility.

▶ **A FULL CAMPAIGN IN ONE MISSION**

A dense, 1-2 hour layered operation inside the infected County Hospital. This is not a short demo—it's a substantial cooperative raid with progression, puzzles, and a climactic boss fight.

▶ **THE TRAITOR PROTOCOL**

After the final boss falls, the real experiment begins. One random player is transformed into a giant spider with unique abilities, turning the game into a tense **PvP hunt**.

If the spider is killed, it respawns as a human while an AI-controlled spider takes over, infecting new players upon contact. The threat never dies—it **evolves**.
