A pixel-art doctor simulator in a run-down Soviet-style clinic. Question patients, examine them, read their charts and spot forged referrals. Then prescribe, refer or refuse, and sign and stamp it yourself while the clock keeps ticking. Most patients don't need what they ask for.

About This Game

Welcome to Profilaktinsk

Six years of med school, paid for with a loan. Then a new law sends you to Clinic No. 67 in Profilaktinsk for three years of mandatory service. Refuse, and you lose your accreditation. Welcome.

Patients walk through your door every day. Some have a runny nose. Some bring a forged referral for opioids. Some are a grandmother who wants the drops she saw advertised on TV. Most of them don't need what they're asking for.

Your job is to figure out who's actually sick, who's lying, and what they should get: a prescription, a referral, a certificate, or a refusal.

What You'll Do

  • Read patient charts — allergies, chronic conditions, current medications. Skip a line and you'll prescribe something dangerous.

  • Examine by hand — choose what to check: temperature, throat, lymph nodes, chest, blood pressure, abdomen, lower back, skin, eyes. Every check costs minutes. Compare what they say with what you find.

  • Verify documents — some referrals are real, some have a fake stamp, an expired date, or the wrong license format.

  • Decide — prescribe, refer to a specialist, issue a certificate, or refuse. Sign it with the pen and stamp it yourself. Every decision affects your rating.

  • Watch the clock — one hour of appointments a day, 12 minutes per patient. Finish early and the front desk sends patients without a ticket. Fall behind and booked patients leave and file a complaint.

  • 5 working days, up to 40 patients. Every morning the head of the clinic posts a new order: new drugs, new specialists, new rules

  • 10 interface languages: English, Russian, German, Spanish, French, Italian, Portuguese, Czech, Japanese, Chinese

  • A reference book on your desk, printed in 1984: drugs, interactions, lab values, symptoms, forms

  • An AI medical assistant on your phone that hallucinates 30% of the time. Use at your own risk.

  • A fixed salary plus a bonus for your rating. At the end of the day patients review you on the city health portal — not always fairly

  • Nurse Lena walks you through your first patient

The Full Game

Three years of mandatory service, played as six weeks several months apart. Patients come back: if you got it wrong, with the same problem; if you got it right, sometimes with a gift. Some of them you'll see for all three years, and how it ends for them depends on what you prescribe. Several endings, from getting fired to being asked to stay.
