The game shows lusty women in a cult who have fallen victim to the void which is a dark dimension that enhances the women's bodies and filling them with lustful desires. Characters range from fully nude, to partially nude. All the characters have extremely sexulized bodies with extreme curves.

Unlocked animations can show erotic, sexual behavior and or sexual acts like masturbation. Captive women are found throughout the game and must be released. The captive women are nude chained and restrained.

Some of the content could also be characterized as non-consensual sex, body modification, BDSM, brain washing and forced masturbation. Some characters have there nipples pierced with chains and other decorative items on there breasts. There is also body painting and text that is on some of the characters during the unlocked model viewing content.

Violence in the game is based on the spells you collect. There is burning, body explosions, blood, and the use of a whip that causes the enemy to bleed.

Smoking can be seen from the main character during conversations and there is use of fowl language.

Play as detective Max washed ashore on a dark nefarious cult island. Your ship has sunk you must explore the island and Black Blood Castle to unravel it's mysteries. Defeat the erotic, sexualized and voluptuous cult women that follow the calling of the Void. Save the missing women and escape the island.

Cult 87 plays like an old school shooter with 2D and 3D elements. The 2D elements use a 8 sided sprite system to give you the feeling of 3D like Doom. Some actions require a selection to the storyline giving you some freedom to choose what you want to say or interact with.
