Hidden behind the walls of a small municipal hospital lies one of the government’s darkest secrets. When an extraterrestrial organism crashed to Earth, it was taken to a covert facility beneath the County Hospital for study. Years later, the entire complex has gone silent. You are part of a special containment unit sent to uncover the truth—and stop whatever is still alive down there.

County Hospital Cautery is a co-op and single-player sci-fi horror experience for up to four players, set in a world where science has gone too far. Equipped with isolation suits, oxygen masks, and industrial flamethrowers, your team must descend into the depths of a decaying hospital and a hidden laboratory where unspeakable experiments fused human tissue with alien DNA. What began as research soon became a feeding ground for an unknown creature.

Every descent into the hospital brings new horrors

The halls are covered in living flesh, the ceilings drip with muscle and sinew, and twisted mutants roam freely—abominations crafted from human and animal DNA. Massive arachnid creatures stalk the corridors, guided by the hive intelligence of the original organism buried deep below a rocket testing site in the lab, How convenient!.

Survival demands teamwork, precision, and control of your resources. Manage your oxygen, stamina, and flamethrower fuel carefully; each is essential to survival. The massive mobile recharge unit outside the hospital restores your life energy and equipment, but venturing back to it means risking what waits in the dark. Inside, medkits and gas canisters can be scavenged to extend your chances—but every sound, every flicker of light, can draw the infected closer.

Your ultimate objective: infiltrate the underground laboratory, ignite every remaining specimen, and destroy the colossal core that once consumed hundreds of lives. But there’s one last threat. The infection spreads silently, and one of your teammates might already be infected. When the transformation begins, they will turn into a monstrous arachnid and hunt the others until destroyed.

GAMEPLAY FEATURES 

  • Flamethrower Fuel System – The flamethrower uses a limited gas supply that decreases with every use. Players must monitor their fuel level and refill periodically at the external recharge unit or by finding portable gas canisters inside the facility. Running out of gas leaves the player defenseless.

  • Stamina & Energy Management – Sprinting and certain actions consume stamina, which regenerates slowly over time. Energy recovery is significantly faster when using the mobile recharge machine located outside the hospital. Managing stamina effectively is essential for survival, especially during chases or escape situations.

  • Life Energy Recharge – Player health (life energy) can be restored at the recharge unit or by locating hidden medkits throughout the map. Damage from enemies or environmental hazards requires frequent monitoring of suit integrity.

  • Recharge Station Mechanics – The large mobile energy trailer functions as a central hub where players can fully restore gas, stamina, and health. However, using it requires leaving the hospital temporarily, introducing risk and strategic timing to every mission.

  • Scavenging System – Essential resources such as gas canisters and medkits are randomly placed across the environment, encouraging exploration and team coordination. These items are limited and do not respawn, increasing difficulty as the mission progresses.

  • Transformation Mechanic (Infection System) – One player can become infected during gameplay. The infected player transforms into a mutated arachnid, gaining unique movement abilities such as faster speed, silent climbing, and unlimited stamina. The transformed player loses access to human equipment but gains enhanced melee attacks and stealth capabilities.

  • Player Balance & Strategy – Human players depend on limited fuel and stamina, while the infected arachnid has unrestricted mobility. This asymmetry creates a balance between resource management and aggressive play, encouraging coordination and constant awareness.

  • Cooperative Gameplay Design – The game supports 1–4 players. Resource management, timing, and teamwork are critical. Players must plan recharge cycles, share supplies, and cover each other while completing objectives inside the contaminated facility.
